Chief Executive Officer
Chuck Smith joined Encompass Rehabilitation Hospital of Utah with more than 25 years of post-acute experience. He began his healthcare career as a floor nurse. He continued to grow his skill as a manager, director, financial officer, regional operations director and as VP of hospital services prior to joining the Encompass Health team. In addition to his nursing, Chuck holds a bachelor’s degree in business administration & management and a master’s degree in business administration. He enjoys building strong hospital teams that work together in providing excellent patient care.
Medical Director
Dr. Koniuch completed medical school in Detroit, MI at Wayne State University, an internship at Henry Ford Hospital, and a Physical Medicine & Rehabilitation residency at Schwab Rehabilitation Hospital in Chicago, IL. He has been with Encompass Health since 2016. During that time he has served as medical staff president, director of rehabilitation and medical director. He has implemented a monthly grand rounds lecture series, developed a gait & balance program in the therapy department and was awarded the CPR award. He treats all patients as if they were family members and always strives for quality care.
Dr. Koniuch is an independent private practice physician.
Director of Pharmacy
Paula is an accomplished pharmacist who brings over 20 years of pharmacy experience and more than 15 years in leadership to her role of director of pharmacy. She earned her Doctor of Pharmacy from the University of Utah College of Pharmacy in Salt Lake City, Utah. Since joining Encompass Health Rehabilitation of Utah in 2009, Paula has played a pivotal role in enhancing medication safety and ensuring high levels of patient satisfaction. Her career began in retail pharmacy and advanced into hospital pharmacy, where she developed extensive expertise in delivering exceptional medication therapy and education to patients both in hospital settings and within the community.
Chief Nursing Officer
With over 20 years of experience in nursing, Jordan has spent nearly 19 of those years at Encompass Health, where she has developed a distinguished career. Starting as a nursing assistant and advancing through roles as a licensed practical nurse (LPN), registered nurse (RN) and Certified Rehabilitation Registered Nurse (CRRN), Jordan has built a comprehensive expertise in rehabilitation nursing. Having held various roles throughout her career, Jordan possesses a deep understanding of each position and how they interconnect. This extensive experience allows her to approach patient care with a well-rounded perspective and an integrated approach to rehabilitation. For the past 15 years, Jordan has specialized in rehabilitation nursing, finding immense fulfillment in guiding patients through their recovery journeys. The opportunity to witness patients regain their strength and return home has been a cornerstone of Jordan’s career, highlighting her commitment and skill in this field.
Facilities Management Director
Rulon has over 35 years of experience in building maintenance with 23 years at Encompass Health. Rulon ensures that our patients and employees are in a safe, clean, healthy environment by maintaining and caring for our hospital facility. Our Encompass Health hospitals maintain the highest standards and is accredited by The Joint Commission.
Human Resources Director
Natalie has been a human resources professional for 17 years. She began her career in the product development and retail industries and has been in the healthcare industry for the last 11 years. Prior to joining Encompass Health Rehabilitation Hospital of Utah, she was a human resources director at a long-term acute care hospital and a behavioral health hospital. She received her bachelor's degree in social sciences from Brigham Young University. She enjoys helping to build strong teams through the implementation of employee and management processes.
Controller
Cora is a seasoned professional with an extensive background in healthcare finance, boasting 20 years of industry experience. Her expertise lies in managing financial operations and ensuring fiscal stability within healthcare organizations. Alongside her successful career, Cora finds joy and balance in pursuing her passions for Pilates and golf. When she’s not crunching numbers or analyzing financial data, you can find her on the Pilates mat or enjoying a round of golf. Cora’s dedication, knowledge and commitment to both her professional and personal pursuits make her a well-rounded individual who continually strives for excellence in all aspects of her life.
Business Development Director
Robin joined Encompass Health Corporation in 2018 with 33 years of experience in various areas of healthcare. Throughout her career, she has had the privilege of providing compassionate care and making a positive impact on countless lives. Beyond the hospital walls, she finds joy in exploring the world through travel and nurturing her hobby farm. Whether she’s tending to patients or tending to her land, her passion for helping others and cultivating a nurturing environment remains at the forefront of everything she does.
Director of Case Management
Liz graduated from the University of Utah with a bachelor's degree in social work. She found a love for social work when she traveled the world for a year with a humanitarian group, enriching her understanding of diverse cultures and communities. Alongside her passion for social work, she enjoys musical arts, playing piano, organ and the ukulele. Liz is dedicated to helping patients and takes pride in being a member of the team at Encompass Health Rehabilitation Hospital of Utah.
Director of Quality & Risk Management
Connie has worked for Encompass Health of Utah for over 14 years. She received her bachelor's degree from Utah State University and her master's degree in physical therapy from the University of Utah. Having worked as a physical therapist and manager in the medical field for over 20 years, Connie has a passion for giving quality care. She thrives on making sure that both patients and staff are respected and can succeed and progress in a positive and safe environment. In her free time, Connie enjoys traveling and camping, woodworking projects and creating a little homestead for her family.
Director of Therapy Operations
Wendy has been practicing occupational therapy for more than 30 years. She received her bachelor’s degree in occupational therapy from the University of Southern California in 1992. Wendy has worked in multiple settings including acute care/intensive care, skilled nursing, home health, NICU, as well as inpatient rehabilitation. She’s a member of the American Occupational Therapy Association. Wendy’s passion is returning dignity and independence back into the lives of our patients and creating an environment for therapists to thrive.
