Our goal is to deliver high-quality care to patients, families and caregivers where it matters most – at home.
Encompass Health is a national sponsor of Together to End Stroke
Serving the Fayetteville, AR area, we are one of the nation’s leading providers of home health services and continually set the standard of homecare through our people, our approach and our outcomes.
The patient experience is at the core of everything we do. That’s why we work collaboratively with the patient’s team of experts to craft a plan of care that meets their specific needs. Our skilled nurses, physical therapists, occupational therapists, speech language pathologists, medical social workers and home health aides use a coordinated, interprofessional approach to deliver compassionate, specialized care in the comfort of home.
Whether recovering from a surgery, recent hospital stay, or managing a disease or injury, our services are designed to meet patient’s where they are. We offer cutting-edge specialty programs that focus on patient education, self-management and reducing hospital readmission, resulting in better care and better outcomes for our patients.
Our Post-Operative Programs, including Orthopedics, Heart & Vascular and General Surgery, deliver protocol-driven, physician-specific care to surgical patients. We admit patients timely after a hospital or facility discharge and provide services daily if needed. We educate patients prior to surgery so they know what to expect throughout their recovery journey.
An estimated 85% of falls occur at home, and 44% of these are due to environmental hazards. Our programs use a multifactorial assessment to identify fall risk and develop an individualized plan of care that significantly reduces the risk of falls. We incorporate evidence-based practices along with outcome reports to help our patients achieve superior outcomes.
Our Cardiopulmonary Programs, including Heart Failure, Chronic Obstructive Pulmonary Disease (COPD) and Hypertension, utilize standards of practice incorporated into our clinical pathways and educational materials. Our programs focus on patient education, self-management and compliance, resulting in exceptional patient outcomes and a reduced need for emergency care.
Through our Community Care Program, we are able to provide a better way to care to patients living in assisted or independent living facilities, as well as residential care communities. We take a coordinated approach to care that allows patients to safely age in place.
Our Care Transitions Program focuses on continuity of care between the patient’s team of experts to provide a safe transition home. This program ensures all the patient’s needs are met so they can achieve the best possible outcomes.
Our Care Connections Program empowers patients to remain in control of their healthcare decisions while providing a seamless transition from home health to hospice. This program seeks to improve quality of life by providing the appropriate level of care that is overseen by one company.
