Our hospice medical directors, in coordination with your physician, create a customized care plan to guide our hospice clinicians, resulting in exceptional, compassionate end-of-life care. Our hospice care services are accessible by phone 24 hours a day, 7 days a week. In addition to scheduled visits, we provide care and support on an emergency basis, including crisis care situations to help patients, where possible, avoid having to go to an emergency room.
Our hospice team helps manage pain and symptoms with the goal of minimizing or eliminating discomfort. We support patient care with medication therapy and medical equipment. Our skilled hospice team delivers support, counseling, companionship, spiritual guidance and bereavement care in addition to medical services.
