Your Rehabilitation, Your Choice

After a hospital stay, your physician or case manager may recommend you transition to another care setting as part of your recovery. These settings—known as post-acute care—can look very different. Should you need additional care, your care team could refer you based on your abilities and goals, but as a patient, you have a choice in which provider you or your loved one go to. It's your right as a patient to choose the provider that best fits your unique abilities and goals for getting better.

Your rehabilitation choice

You make decisions every day. The question is, are you making the right ones?
We know how important it is to ask smart questions to help you or your loved one make the right decision for rehabilitation.
Questions like:

  • Will a physician see me daily?
  • What technologies are used in therapy?
  • Are nurses certified in rehabilitation?
  • Am I guaranteed at least 15 hours of therapy per week?

Smart questions lead to smart decisions. Ask for Encompass Health hospitals by name.
