Fall risk model

In 2021, we initiated our fall prevention model, which combines predictive modeling with our core clinical practice protocols. Since then, we have seen our fall rates per 1,000 patient days improve by 30%.

Readmission prediction model

Implemented in 2020, this model predicts a patient’s probability of a hospital readmission after they leave the rehabilitation hospital.
