When you choose West Tennessee Healthcare Rehabilitation Hospital Cane Creek, a partnership with Encompass Health for your rehabilitation, you receive compassionate care from an expert team of physical, occupational and speech therapists; physicians; nurses; dietitians; pharmacists; and case managers who work together to create a plan to help you meet your unique goals.

At our hospital you will receive:

  • 24/7 nursing care, including nurses who specialize in rehabilitation

Jonathan McAnulty, OTR/MBA

Chief Executive Officer

Jonathan McAnulty serves as the chief executive officer at West Tennessee Healthcare Rehabilitation Hospital Cane Creek in Martin, TN.

Jonathan McAnulty serves as the chief executive officer at West Tennessee Healthcare Rehabilitation Hospital Cane Creek in Martin, TN. He joined Encompass Health in 2017 as the director of quality and risk management in Jackson, TN. During this time, he completed his Masters of Business Administration from Bethel University in 2020. Receiving his bachelor's degree in health and human performance in 2005 from the University of Tennessee at Martin. Jonathan began his healthcare career in marketing and medical sales. In 2011, he earned his Masters of Occupational Therapy from the Tennessee State University. Jonathan began his occupational therapy career at West TN Rehabilitation Center as the lead therapist on the inpatient spinal cord injury rehabilitation team while also serving in acute care, neonatal and outpatient rehabilitation settings.

Medical Director

Dr. Miller has practiced internal medicine for more than 25 years in a variety of healthcare settings.

Dr. Miller has practiced internal medicine for more than 25 years in a variety of healthcare settings. He received his medical degree from East Tennessee State University, he has spent time treating patients in Guam, stateside in rural areas of private practice and multiple acute care hospital settings. His hobbies include owning and operating Blackberry Pond Farms which offers organic vegetables grown hydroponically to the rural West Tennessee community.

Candace Castleman, RN

Director of Quality and Risk Management

Candace joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek in 2009 as a rehabilitation liaison.

Candace joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek in 2009 as a rehabilitation liaison to local community hospitals and physicians. She became director of quality and risk management in 2013. Candace completed her Bachelor of Science in Nursing from the University of Tennessee at Martin in 2008 and became a CRRN in 2013.  Candace heads the hospital’s patient satisfaction initiatives, quality reviews, and advanced certification requirements including The Joint Commission and National Stroke Association. Candace is a native of Greenfield, Tennessee and currently resides in Martin, Tennessee with husband, Bryan and their son, Wade.

Greg Grooms

Facilities Management Director

Greg joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek in October 2011.

Greg joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek in October 2011. Greg began his career as a maintenance mechanic doing general facility maintenance. He was promoted to maintenance manager in August 2013 and is now serving as the hospital’s director of plant operations, for which he oversees environmental care and life safety for the hospital. This involves establishing and following preventative maintenance programs as well as meeting standards for The Joint Commission. Greg received his Certificate in Industrial Maintenance from Tennessee Technology Center in Newbern in 2007. He is a retired staff sergeant with the Tennessee National Guard with 25 years of service.

Director of Case Management

Shala joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek in February 2016 as a case manager.

Shala joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek in February 2016 as a case manager. She previously worked as a social worker director for a local skilled nursing facility. Prior to her role as a director, she worked as a resource advocate for the State Health Insurance Program, of Medicare and Medicaid Services for the State of Tennessee. She graduated from the University of Tennessee at Martin in 2013 with a bachelor’s degree in social work and has recently received her certification as ACM-SW. In her role at Cane Creek, Shala is responsible for overseeing the discharge process by collaborating with the interdisciplinary team, and coordinating with families for a smooth transition to the next level of care.

Lori Manley, OTR/L

Director of Therapy Operations

Lori has been a pivotal component of West Tennessee Healthcare Rehabilitation Hospital Cane Creek’s therapeutic rehabilitation programs for more than 20 years.

A native of Osceola, Arkansas, Lori has been a pivotal component of West Tennessee Healthcare Rehabilitation Hospital Cane Creek’s therapeutic rehabilitation programs for more than 20 years. She holds a Bachelor of Science in Occupational Therapy from the University of Central Arkansas and has served in numerous leadership roles at Cane Creek. Lori exemplifies the value of continuing education for her staff by achieving the rank of clinical specialist II through Encompass Health’s clinical career ladder. She has earned advanced certification in Neuro Developmental Treatment (NDT) and Interactive Metronome.  As director of therapy operations, Lori is responsible for all the therapy disciplines and spearheads several patient education initiatives in the hospital.

Sharon has been director of human resources at West Tennessee Healthcare Rehabilitation Hospital Cane Creek since 2007.

Sharon has been director of human resources at West Tennessee Healthcare Rehabilitation Hospital Cane Creek since 2007. Prior to joining Cane Creek, Sharon held human resource leadership positions at MTD Products for several years. In her role at the hospital, Sharon is responsible for recruiting new talent, coordinating our professional student programs for nursing and therapy, and various employee and community events.

Kristin Skinner

Chief Nursing Officer

Kristin joined the staff of Rehabilitation Hospital Cane Creek, in 2013 as a registered nurse and became nurse manager in 2014.

Kristin joined the staff of West Tennessee Healthcare Rehabilitation Hospital Cane Creek, in 2013 as a registered nurse and became nurse manager in 2014. Kristin also served as infection preventionist, employee health coordinator and hospital educator during her tenure at Cane Creek. Kristin obtained an associate’s degree in nursing from Dyersburg State Community College in 2012, certified rehabilitation registered nurse in 2015 and a bachelor’s degree in nursing from South University in 2018. As chief nursing officer, Kristin is responsible for developing and implementing programs enhancing a culture of safety and accountability for all aspects of patient care and supervising and coordinating nursing personnel.

Director of Pharmacy

Emily obtained her bachelor’s degree in cellular and molecular biology from the University of Tennessee at Martin in 2012.

Emily obtained her bachelor’s degree in cellular and molecular biology from the University of Tennessee at Martin in 2012. She then attended Union University School of Pharmacy and received her Doctorate of Pharmacy in 2016. Prior to joining West Tennessee Healthcare Rehabilitation Hospital Cane Creek, Emily was a retail pharmacist. In her role at Cane Creek, she is responsible for medications throughout the hospital as well as patient education.

Christy joined West Tennessee Healthcare Rehabilitation Hospital Cane Creek as the controller in January 2024.

Christy joined West Tennessee Healthcare Rehabilitation Hospital Cane Creek as the controller in January 2024. Prior to Encompass Health, Christy served in acute care and clinical settings and brings more than 19 years of accounting and financial management experience to her role. Christy graduated from the University of Tennessee at Martin in 1998 with a bachelor’s degree in accounting.

A member of the team since 1989, Jan has been instrumental in the hospital’s growth and development over the years.

A member of West Tennessee Healthcare Rehabilitation Hospital Cane Creek’s team since 1989, Jan has been instrumental in the hospital’s growth and development over the years. A certified coder, Jan is a resource assisting other Encompass Health hospitals with various needs nationwide. As director of health information management at Cane Creek, Jan leads our team in ensuring completeness of health records, coding compliance and privacy of protected health information.

Business Development Director

Jada joined the staff at West Tennessee Healthcare Rehabilitation Hospital Cane Creek in 2014.

Jada joined the staff at West Tennessee Healthcare Rehabilitation Hospital Cane Creek in 2014. During her time with Cane Creek, she has served as a registered nurse, rehab liaison and admissions liaison before being promoted to director of marketing operations in 2017. She holds a bachelor's degree in health and human performance from the University of Tennessee at Martin as well as an associate degree in nursing from Jackson State. In her role at Cane Creek Jada is responsible for the implementation of the referral process, oversees hospital admissions, as well as marketing and business development.
