Leanne was announced as interim CEO of Encompass Health Cape Coral in May 2024. Prior to joining the Encompass Health team, Dr. Salazar served as the chief nurse executive for HCA West Florida Division for over four years. Leanne also served in numerous executive roles at HCA Florida Oak Hill Hospital including chief operating officer, chief nursing officer and vice president of quality and risk management. Prior to her career move to HCA, Leanne spent 16 years at Spring Hill Regional Hospital in nursing leadership. Leanne is an advanced registered nurse practitioner with her Doctorate of Nursing Practice (DNP), a master's degree in nursing, a master's degree in healthcare administration and a bachelor's degree in nursing. Leanne is also Board Certified in family practice (FNP-BC), Certified in Executive Nursing Practice (CENP), Certified in Professional Health Care Quality (CPHQ) and a Certified Professional in Patient Safety (CPPS).

Dr. Soofia Khan is the medical director at Encompass Health Rehabilitation Hospital of Cape Coral and is committed to bringing her expertise to support the community. She is board-certified in PM&R, pain medicine and fellowship-trained in spinal cord injury medicine.

She is focused on optimizing function, minimizing pain and enhancing quality of life through a multidisciplinary, patient-centered approach. She believes in collaborating with primary care providers/internists, neurologists, surgeons and other specialists to ensure seamless, comprehensive care to each patient’s unique needs and goals. She has a strong foundation in diagnosing and treating a wide range of conditions, including musculoskeletal injuries, neurological disorders and pain results in better outcomes.

She received her postgraduate degree from Tufts Medical Center and completed her PM&R residency at Tufts University in 2003. She completed a Spinal Cord Injury Medicine Fellowship at University of Miami, Miller School of Medicine and has since focused on clinical/academic rehabilitation and pain management pursuits. She has extensive knowledge of the Spinal Cord Injury Model System (SCIMS) and has been involved with national spinal cord injury organizations and clinical practice guidelines. With a proven track record as a medical director, she has spearheaded diverse inpatient rehabilitation programs specializing in general rehabilitation, SCI/D and functional restoration.

Dr. Khan is an independent private practice physician.

Marianita “Mamet” Allen serves as the director case manager for Encompass Health Rehabilitation Hospital of Cape Coral. She earned her master’s degree in social work from the University of Washington and a bachelor’s degree in social work from St Theresa’s College in the Philippines. She is a board-certified case manager (CCM) and is a licensed clinical social worker (LCSW). Prior to coming to Encompass Health Cape Coral, she spent more than seven years at Encompass Health Rehabilitation Hospital of Sarasota. Prior to her career move to Encompass Health, Marianita spent over 20 years in acute and post-acute care as a medical social worker and case manager.

Kristy brings a wealth of knowledge with more than 20 years of experience in clinical and nursing operations. Kristy has successfully driven effective strategies which have enhanced nursing practice, optimized patient outcomes, elevated patient experience and improved nursing engagement. Kristy most recently served as the chief nursing officer at HCA Florida Fawcett Hospital where she improved nursing retention and patient experience. Prior to joining Fawcett, she served as the assistant chief nursing officer at Lewisgale Medical Center in Salem, Virginia where she improved patient throughput, quality measures and productivity. Most notably, Kristy spent over 20 years in the Lee Health System with most of her tenure as a system director of nursing. In that role, she developed and managed a system-wide culture of safety program and received the lifetime achievement award for employee engagement. She holds a Master of Science in nursing leadership from Walden University and a bachelor’s degree from Nova Southeastern University.

Gaston serves as the director of therapy operations at Encompass Health Rehabilitation Hospital of Cape Coral. A licensed physical therapist for almost two decades with seven years of leadership experience, he joined Encompass Health Deaconess Rehabilitation Hospital in Evansville, Indiana in April 2014. He obtained his bachelor's degree from Universidad Andres Bello, a master's degree in physical therapy from Universidad Andres Bello and a master's degree in business management from Universidad Adolfo Ibanez. Most recently, Gaston earned a doctorate degree in physical therapy from Shenandoah University in 2020.

Chea serves as director of quality and risk at Encompass Health Cape Coral. She brings more than 25 years of healthcare experience to the team including over five years of risk management. An Indiana native, she graduated with a bachelor’s degree in nursing from Ball State University. Chea is also certified as a professional in healthcare risk management with the American Hospital Association.

Sophia serves as director of pharmacy at Encompass Health Cape Coral. Sophia brings more than 10 years of pharmacy experience to the team with time spent at ShorePoint Health in Port Charlotte and Physicians Regional Medical Center. She completed her pharmacy residency in infectious diseases from Stoney Brook University Medical Center in Stoney Brook, New York and her clinical pharmacy residency from Upstate Medical University Hospital in Syracuse, New York. Sophia earned her doctor of pharmacy degree from Florida A&M University.

Marc joins Encompass Health Rehabilitation Hospital of Cape Coral as facilities management director. He previously held the same role at our Encompass Health Rehabilitation Hospital of York in York, Pennsylvania. Marc brings more than 20 years combined experience in residential and commercial construction management to his role. In addition, Marc has management experience in research and development and logistics operations.

Stacy brings over 10 years of human resources healthcare experience and held various leadership roles during the span of her career. Stacy received a bachelor’s degree in psychology from the University of Central Florida, and later went on to complete the human resource development master’s program at Villanova University. Stacy obtained certifications from the Society for Human Resource Management, Prosci Change Management and Coaches Alliance as a leadership coach. In addition to these obtained certifications, Stacy is also trained as a DiSC facilitator and has helped numerous leaders develop and hone their professional abilities. Stacy is passionate about setting goals that have a positive impact on organizational culture while focusing on key drivers of engagement to build high performing teams.

Larry brings more than 40 years of accounting experience to our Encompass Health Cape Coral team with over 20 years of healthcare finance experience. Most recently, Larry served as controller for one of Florida’s largest physician practice and surgery center. Larry earned his master’s degree in accounting from St. Thomas University and bachelor’s degree in business administration from Florida International University. He is a member of the Florida Institute of Certified Public Accountants and American Institute of Certified Public Accountants.

Diana serves as the business development director of Encompass Health Rehabilitation Hospital of Cape Coral. Diana has 30 years of experience in the healthcare market with a vast knowledge of healthcare business, professional development and clinical services. Prior to joining Encompass Health Rehabilitation Hospital of North Tampa in 2021, she worked for Northwestern Medicine Marianjoy Rehabilitation Hospital as an occupational therapist and then transitioned to clinical rehabilitation liaison. Drawing on her healthcare and marketing experience, she leads a team to identify care needs for our patients and healthcare community.
