We will work with you to educate you and your loved ones on how to manage your diabetes so you may prevent future health complications. Our expert care team will help manage your blood sugar, prevent complications and support your overall recovery.

What’s included in diabetes management?

Medication management
Your care team will work with your doctor to ensure you’re on the right medications. Proper medication management can improve your blood sugar and help prevent complications.
Diet and nutrition support
Our registered dietitians and occupational therapists work together to ensure your meals meet your dietary needs. They will also teach you about portion sizes, food choices and meal timing to keep your blood sugar stable.
Exercise
Therapists will guide you through safe, daily physical activity to help improve strength, mobility and blood sugar control.
Wound care
If you have wounds or surgical incisions, our wound care specialists will monitor healing and teach you and your family how to care for them safely at home.
