At Encompass Health Rehabilitation Hospital of Cypress, we are committed to helping you regain independence after a life-changing illness or injury. Located in Cypress, we serve the greater Houston area as a leading provider of pulmonary rehabilitation, inpatient physical rehabilitation for stroke and amputation and other complex neurological and orthopedic conditions.
Using clinical collaboration and advanced technologies, we provide a personalized care plan designed to meet your unique needs and help you achieve your goals.
Our inpatient rehabilitation hospital uses an interdisciplinary team approach that includes physical, speech and occupational therapists, rehabilitation physicians, rehabilitation nurses, case managers, dietitians and more, combined with our advanced technology and expertise, to help you achieve your goals. You will receive at least three hours of therapy five days per week while under the constant care of registered nurses, many of whom specialize in rehabilitation, and frequent independent private practice physician* visits.
Rehabilitation is covered in full or part by most health insurance plans, including Medicare and Medicaid. Our hospital has contracts with most major insurance companies and is willing to negotiate with other companies, as needed.

Chief Executive Officer
Sheila serves as chief executive officer for Encompass Health Rehabilitation Hospital of Cypress. She joined Encompass Health in March 2010, as the interim chief executive officer at Encompass Health Rehabilitation Hospital of Sugar Land. Sheila has a bachelor’s degree in general education as well as respiratory therapy. An accomplished healthcare professional with 21 years experience in the industry, she has a strong background encompassing all aspects of business development and operations in the post acute care environment.
Medical Director
Dr. Lachina has served as the medical director of Encompass Health Rehabilitation Hospital of Cypress since October 2011. He completed his medical studies at Universidad Autonoma de Guadalajara and completed his residency training in physical medicine and rehabilitation at New York Medical College.
Prior to joining HealthSouth, Dr. Lachina served as the medical director for the rehabilitation department at Memorial Hermann Northwest Hospital for 22 years. He is certified by the American Board of Physical Medicine and Rehabilitation and is a member of the Harris County Medical Society, Texas Medical Association and American Academy of Physical Medicine and Rehabilitation.
Dr. Lachina is an independent private practice physician.
HIMS Supervisor
Candi has been with Encompass Health Rehabilitation Hospital of Cypress for five years. For the past two years, she has held the position of HIMS supervisor/hospital HIPAA officer. She is pursuing her RHIT associate's degree and sims to finish in 2022. She ensures the hospital follows HIPAA regulations and guidelines, particularly the security of protected health information (PHI).
Controller
Melissa Haddox joined the Encompass Health team as controller in March, 2013. She is a Certified Public Accountant and has a bachelor’s degree in accounting from West Texas State University. Melissa has over 30 years of experience in healthcare and financial management. Previously, she has worked at for profit hospital corporate offices in management roles with responsibility for Medicare and Medicare reimbursement and regulatory compliance in both inpatient acute and post-acute settings.
Director of Quality and Risk Management
Ashley joined Encompass Health Rehabilitation Hospital of Cypress in June 2012. Her prior work experience includes data analysis in a risk management department for a non-profit medical organization. She then moved to the hospital setting in order to gain experience in quality management, which includes patient satisfaction, process improvement, and compliance. Ashley received her bachelor’s degree in communication from St. Edward’s University in 2006 and her MBA from University of Houston – Victoria in 2012.
Director of Pharmacy
Melinda has been a pharmacist for more than 11 years with a background in acute care, ambulatory care and outpatient pharmacy. She graduated with her doctorate of pharmacy degree from Texas Southern University in 2010 and is currently pursuing a degree in master of business administration with a concentration in healthcare administration from Texas A&M University. She was a pharmacy student for Encompass Health and later joined the team as a licensed pharmacist in 2010. She is committed to improving patient outcomes and seeing them meet their goals.
Area Business Development Director
Tamara has more than 25 years of experience in healthcare, most notably, over 10 years with Encompass Health Rehabilitation Hospitals. Her background has been in pharmaceutical sales and hospital business development. She previously served as business development director at Encompass Health Midland Odessa before moving to northwest Houston. A native of west Texas, Tamara earned her bachelor’s degree in communications from Texas A&M University.
Director of Facility Management
Alex serves as the director of facility management for Encompass Health Rehabilitation of Cypress. Alex joined the Cypress team in 2018 after working nine years in facility management for an acute care hospital. He received his bachelor's degree in mechanical engineering from Baylor University in 2009. Alex is responsible for environmental services, maintenance and life safety of the hospital.
Director of Human Resources
Byron serves as the director of human resources for Encompass Health Rehabilitation Hospital of Cypress. He is a graduate of Texas A&M University. He joined the Encompass Health Cypress team in 2016 after spending the previous 12 years in the acute care hospital environment. Byron is responsible to plan, organize and direct all aspects of human resources management for the hospital.

Chris has been employed at Encompass Health for 8 years.
Chris has been employed at Encompass Health for 8 years. She graduated from Southern Illinois University Edwardsville with a bachelor’s of science degree in nursing. She is a Certified Case Manager.
Director of Therapy Operations
Susha serves as the director of therapy operations at Encompass Health Rehabilitation Hospital of Cypress. She joined Encompass Health in November 2014 as a physical therapist and was promoted to physical therapy team lead soon after. Prior to joining Encompass Health in 2014, Susha worked in various settings including acute inpatient, home health and skilled nursing. Susha has been a physical therapist since 2003 and received her Doctor of Physical Therapy from Wayne State University in Michigan. She has specializations in neurological rehabilitation including C/NDT for stroke rehabilitation and LSVT BIG for Parkinson’s rehabilitation. Susha is passionate about providing exceptional patient care and returning patients to independence.
