We want you to get better, and to make that happen, we need to know where you began. Upon arrival at one of our hospitals, we will measure how much help you need to perform basic skills – this is called your Functional Outcome Measures. Your rehabilitation team will review these with you, set challenging but attainable goals, and design a treatment plan to help you meet your specific goals. Before you leave the hospital, we will rescore your Functional Outcome Measures assessment to see how much you’ve improved, determine how well you have met your goals, and provide you the materials and training you need to continue your progress after you leave the hospital.
Brain Injury Program Manager
Gianna brings more than two years of clinical experience as an occupational therapist to Encompass Health Rehabilitation Hospital. Gianna started as a rehab tech in 2015. A Toms River native, she graduated from Stockton University with a bachelor’s degree in health science and a minor in holistic health. Gianna also holds a master’s degree in occupational therapy from Stockton University.
Wound Care Program Champion
Shannon joined the hospital team in 2015 and has been with the wound care team since 2017. In 2018, she became the wound care program champion. In addition, she holds certification as a wound care clinician and an ostomy management specialist. Shannon holds a BSN from Rutgers University and is a certified rehabilitation registered nurse.
Stroke Program Manager
Jessica joined the Encompass Health Toms River team in 2009 and has been a senior physical therapist since 2011. She is a board-certified specialist in neurologic physical therapy and takes great pride in helping patients with stroke and other neurologic disorders return to their household and community roles. Jessica also serves as the physical therapy department’s center coordinator of clinical education and enjoys clinical teaching and mentorship. She holds a master’s degree in physical therapy from Quinnipiac University
Advanced Inpatient Diabetes Program Champion
Avegail has 18 years’ experience in acute hospital and rehabilitation settings and holds a bachelor’s degree in nursing from University of Santo Tomas Philippines. In addition, she is a Certified Medical Surgical Registered Nurse and Certified Rehabilitation Registered Nurse. She came to work for Toms River in 2004 and was promoted to a charge nurse position in 2006. In 2016, she became the diabetes champion and point of care coordinator and is a proud to be a member of the Advanced Diabetes Program.
Brain Injury Program Champion
Jess began her career at Encompass Health in 2013 after obtaining her clinical doctorate at Drexel University in Philadelphia. In June 2014, Jess became the physical therapy representative of the Brain Injury Program and took on the role of co-champion in February 2015.
Advanced Inpatient Diabetes Program Manager
Patricia Henk is a senior occupational therapist who has been with Encompass Health Toms River since 2006. She graduated from Stockton University in 2009 with a Master’s of Science degree in occupational therapy. Patti has been a co-champion of the Diabetes Program since 2012 and helped the hospital achieve the distinction of Advanced Certification for Inpatient Diabetes in 2014 from the Joint Commission. Her interest is in education, and she has developed a diabetes education group for our patients to attend.
Wound Care Program Manager
Denise joined the Encompass Health Toms River team in 2012 and has been the wound care coordinator since 2015. She became a certified wound care clinician in 2014 and a certified Lymphedema therapist in 2016. Denise holds a bachelor’s degree from Susquehanna University and a doctorate in physical therapy from Arcadia University.
Cardiac Program Champion
Tricia started at Encompass Health Toms River in June 2012 after graduating from Ocean County College in 2010 with an associate degree in nursing. She is currently attaining her BSN through Chamberlain University School of Nursing. Tricia has been a member of the cardiac recovery program since September 2012 and became the hospital’s Program Champion in January 2015. In June 2014, she passed her Certified Rehabilitation Registered Nurse exam and then moved into the position of north wing charge registered nurse in January 2017.
Stroke Program Champion
Kristen has been a speech pathologist with Encompass Health Toms River since 2007. She received her bachelor's degree in communication sciences and disorders from The University of Connecticut and her master's degree from Montclair State University. Kristen has been an active member of the Stroke Program since 2008.
Our nurses deliver specialized, around-the-clock care that is crucial on a patient’s journey to recovery.
Our physical therapists help patients increase their physical function, mobility, strength, balance and safety by designing specialized testing and exercises.
Our occupational therapists design activity-based therapy to promote a patient’s independence in self-care, home management and work skills.
Our independent private-practice physicians and medical directors have specialized training and experience in rehabilitation medicine, and provide close clinical oversight for each patient and their treatment plan.
Our respiratory therapists help our patients with their lungs and respiratory system when their breathing is affected by injury or surgery.
Our speech-language pathologists help patients overcome communication disorders, speech challenges and swallowing problems.
Our dietary and nutritional counselors oversee each patient’s meals to make sure they receive the proper nutrition during and after treatment.
Our case managers coordinate care plans with our clinical teams, patients and loved ones to ensure all needs are met and any questions are answered.
