Published research also makes it clear—inpatient rehabilitation facilities offer a higher level of care and reduce readmission rates and costs in the long-term. In a first-of-its-kind study, the American Physical Therapy Association (APTA) and the American Occupational Therapy Association (AOTA) showed that physical therapy and occupational therapy provide functional benefits to most patients in post-acute care settings, with evidence of significant rehabilitative value across 1.4 million Medicare cases. And peer reviewed data show that IRFs are the preferred setting for post-acute treatment, including treating hip fractures and hip replacements, stroke and brain injury.
At Encompass Health, we’ve developed and implemented a proprietary, evidence-based clinical initiative to reduce readmissions and improve patient outcomes. In partnership with Cerner, our first predictive model, ReACT, was launched in 2017. ReACT is seamlessly integrated into the clinician workflow and the patient electronic health record (EHR) to identify the risk of an acute care transfer (ACT) in order to prompt clinicians to intervene and adjust the plan of care accordingly.
The ReACT dashboard displays a patient’s ACT risk levels throughout their stay in near real time. The clinical team reviews the dashboard in daily huddles and when rounding. This practice helps focus communication and customize treatment planning and additional assessments, especially for high-risk patients.
No matter what the setting of care, falls present a serious risk to patients and can cause a setback in their recovery. The Joint Commission reports the average increase in a hospital’s operational costs for a serious fall-related injury is more than $13,000, and the average increase in length of stay is about six days. That’s why developing a fall predictive model specific to the rehabilitation setting of care became our latest priority.
Using 400,000 electronic medical records from past Encompass Health patients, this new predictive model identified 54 clinical features that increase a patient’s risk of falling in the inpatient rehabilitation setting. These risk factors include medications, diagnosis, physical and cognitive function and vital signs, to name a few.
The model is now running in all Encompass Health rehabilitation hospitals and begins at admission.
